You Don't Know My Story, Inc. is a 501 (c) 3
Non-Profit Organization.
Our mission is to provide sustainable resources that promote nutrition, education, and community involvement.
We believe that everyone deserves to live a healthy and fulfilling life, and we're committed to making that a reality.
Our primary goal is to rejuvenate low-income communities, firmly believing that authentic love, access to essential resources, health education, robust support, and proper nourishment are the cornerstones of restoration.
Our efforts are directed towards communities affected by Social Determinants of Health (SDOH), recognizing the profound impact of SDOH on people's health, well-being, and overall quality of life.
What Are Social Determinants of Health?
Social determinants of health (SDOH) are the nonmedical factors that influence health outcomes. They are the conditions in which people are born, grow, work, live, and age, and the wider set of forces and systems shaping the conditions of daily life. These forces and systems include economic policies and systems, development agendas, social norms, social policies, racism, climate change, and political systems. Centers for Disease Control and Prevention (CDC) has adopted this SDOH definition from the World Health Organization.
Social determinants of health (SDOH) have a major impact on people’s health, well-being, and quality of life.
Examples of SDOH include:
SDOH also contribute to wide health disparities and inequities. For example, people who don't have access to grocery stores with healthy foods are less likely to have good nutrition. That raises their risk of health conditions like heart disease, diabetes, and obesity — and even lowers life expectancy relative to people who do have access to healthy foods.
Just promoting healthy choices won't eliminate these and other health disparities. Instead, public health organizations and their partners in sectors like education, transportation, and housing need to take action to improve the conditions in people's environments.
